## Fees

#### Private Pay Sessions

| Intake | $200 |

| Individual session | $175 |

Most Flexible
#### Private Pay Concierge Therapy

| Individuals | $600/mo |

| Couples | $800/mo |

#### Insurance-Based Hybrid Concierge

| Individuals | $400/mo |

| Couples | $600/mo |

Fees are due the day of the visit. Cash, check or credit cards accepted online.

## Insurance

Insurance accepted:

- Blue Cross
- Tufts
- OPTUM
- UBH
- HPHC
- MGBH

**If you are out of network, I do not bill insurance but would be happy to provide a superbill for you to submit to your plan.**

If planning to use insurance, please bring your card and co-payment.

### Questions to Ask Your Insurance

- Is mental health covered?
- Is there a deductible and has it been met?
- Is there a copayment and how much is it?
- Does the visit require pre-authorization?
- Do you have out of network benefits?
- What do you need to submit to your insurance in order to be reimbursed for out of network?
- Are any preapprovals required?

## Cancellation Policy

Cancellations and re-scheduled appointments are subject to my full fee $200 intake, $200 a couple or $175 for individuals unless you are Concierge, you can re-schedule the same week, or you have a last minute medical emergency.
